Complex Care Management

Complexity of care increases with severity of disease and comorbidities. The comprehensive condition management for CKD and ESRD patients requires the coordination of primary and specialty care, disease stage-specific interventions targeted to the specific stage of disease, patient education, dialysis monitoring, and management of comorbidities related to kidney failure. Patient activation, caregiver support, behavioral health services, and the coordination of community resources are important for effective disease management. Coordination of care across primary care providers, nephrologists, and other specialists is critical.

Cost of Care
